1) statement;

    2) a copy of the legalized or apostilled document on education and its diploma supplement (the original for verification, is subject to return) and the inherently certified translation (if the document is completely in a foreign language) of the document on education and its diploma supplement, including the translation of the seal.
    When issuing a certificate of education in a country party to an international treaty (agreement) on legal assistance, providing for the abolition of the legalization of official documents to which the Republic of Kazakhstan is a party, legalization or apostilization of it is not required and a notarized copy of the document of education and its diploma supplement will be submitted and notarized certification translation (in case the document is completely in a foreign language).

    3) document proving the identity of the holder of the certificate of education (for identification) or an authorized representative on the basis of a notarized power of attorney.
    For non-residents of the Republic of Kazakhstan, a copy of the document certifying the identity of the holder of the certificate of education (with translation into the state or Russian language).
    If the holder of the education certificate changed the surname, name or patronymic (if available) after receiving it, you must submit a copy of the certificate of state registration of acts of record of changing the name, surname or patronymic (if available) or an act of marriage or divorce;

    4) payment receipt.
