List of required documents for duplicate
To obtain a duplicate of the certificate of recognition of education documents, the following documents shall be submitted:
- When applying to the State Corporation:
1) An identity document and (or) in the form of an electronic document through the digital document service (to identify the identity) of the holder or an authorised representative on the basis of a notarised power of attorney;
2) Application for issuance of a duplicate in the form according to Annex 7 to the Rules;
3) If the holder of the certificate of recognition has changed surname, name or patronymic (if any) after receiving it, it is necessary to submit a copy of the certificate of state registration of acts of recording of change of surname, name or patronymic (if any), or act record on marriage or on dissolution of marriage (with translation into Kazakh or Russian language);
4) The original of the certificate of recognition (if any);
5) For non-residents of the Republic of Kazakhstan - a notarised copy of the document certifying the identity of the owner of the education document (with translation into Kazakh or Russian);
6) Information on payment.
- When applying through the portal:
1) Application for issuance of a duplicate in the form according to Annex 7 to the Rules;
2) If the holder of the certificate of recognition has changed surname, name or patronymic (if any) after its receipt, it is necessary to submit a copy of the certificate of state registration of acts of recording the change of surname, name or patronymic (if any), or act record of marriage or divorce (with translation into Kazakh or Russian);
3) The original of the certificate of recognition (if any);
4) For non-residents of the Republic of Kazakhstan - an electronic copy of the document certifying the identity of the owner of the education document (with translation into Kazakh or Russian);
5) Information on payment.
